Averett Alumni Awards Nomination Form

Alumni, faculty, staff, and friends of alumni are welcome to submit nominations for any of the award categories listed below.

Step 1: Select an award category.

Step 2: Fill out as much of the form as you are able (sections with an * are required). You will be asked to describe and justify why you believe this person is deserving of the award. To improve your success, your nomination should speak to the qualifications of the award.

Step 3: You will be required to submit one or more of the following:

  • Two letters of support for the nomination
  • Curricula vitae or Resume

These letters should show evidence of:

  • Business/professional accomplishments;
  • Community/professional affiliations;
  • Awards, honors or decorations;
  • Publications;
  • Volunteer service to Averett or other organizations.

Step 4: Timelines

  1. Submit nomination forms and other pertinent information to the alumni office by Monday, April 5, 2021.

  3. Award nominations are welcome throughout the year preceding the deadline. They will be saved for consideration for one year.

Other Considerations: The Nominee must be available to attend Homecoming and the Awards Ceremony to receive their award.
