Enrollment/Degree Verification Notification

Forms to be completed & submitted to Registrar’s Office for Processing:

  • Add/Drop Registration
    • Will require approval from student’s academic advisor before processing.
  • Apostille Request
  • Change in Advisor, Major, Minor, or Catalog Request
    • May require approval from student’s academic advisor before processing.
  • Course Substitution
    • Must be submitted and signed by department chair to before submitting to RO for approval.
  • Duplicate Diploma Request
  • Degree Audit (Request for Updated Academic Evaluation)
  • Enrollment Verification
    • Only to be completed by currently enrolled students. Third-party enrollment verifications may be requested via web or by mail. Instructions are listed under Enrollment/Degree Verification Notification at the top of this page. 
    • Former students wishing to obtain their education verification through the Department of Education may reach out with questions to Averett Central at 434-791-5600. Necessary paperwork will need to be completed by Averett’s Education Department and may take a few days for processing.
  • Grade Change Notification
    • Only to be completed by Averett faculty.
  • Independent Study Request
    • Will require confirmation from instructor and advisor before processing.
  • Letter of Degree Completion
  • Name Change/Address Change
  • Pass/Fail Declaration
    • Will require confirmation from instructor and advisor before processing.
  • Permission to Take More Than 18 Credits
    • Will require approval from student’s academic advisor before processing.
  • Request to Take Coursework Elsewhere
    • Only to be completed by currently enrolled students who wish to take courses at another institution and have them transferred back to Averett. Students must have spoken with their academic advisor & meet with a member of the Registrar’s Office to verify the course is directly transferable.
  • Request to take LCMC Course
    • Currently enrolled traditional (on-ground) students wishing to enroll in LCMC course will need to complete this form with their academic advisor & submit to the R.O. for processing.
  • Syllabus Request Form
    • A syllabus for a course is requested using the new Syllabus Request Form. All requests will be routed through the Registrar’s Office. A request for copies of current or archived syllabi can only be made by current or former students or graduates. Copies of syllabi will only be provided for courses the student completed at Averett University based on the student’s academic record.Requests should come via the form for documentation purposes. Please allow at least 7-10 business days for processing of request. If the request is for the purpose of seeking credit for the course at another institution or for employment verification, the Registrar’s Office will research the information requested.

The course and the instructor of record for the course must be identified and confirmed. The Registrar’s Office will do due diligence in researching the course with appropriate offices.  They will work with the Office of Academic Affairs, the instructor, Office of Institutional Research and Effectiveness (OIRE) as well as the archivist if needed to locate syllabi.

Once the syllabus has been located, an email will be returned to the requestor indicating the approval and notification that the syllabus will be sent directly to the Registrar’s Office of the “transfer” institution or to their employer.

The syllabus will be sent to the Registrar’s Office of the “transfer” institution or employer via email. This will be sent as a pdf, and will include student/employee identification information as supplied by the requestor. It will NOT be sent directly to the requestor.

If the request is for something other than obtaining transfer credit at another institution or for employment verification, there may be additional steps required, and the request will be routed through the Office of Academic Affairs.

The Office of Academic Affairs will work with the appropriate instructor (if available) about granting the request. As syllabi are considered the intellectual property of the instructor, they have the final say in approving or denying the request.  The Office of Academic Affairs will email the requestor with the decision once it is determined. Please allow at least 14 business days for a response.

  • Traditional to AU Online Request
    • Currently enrolled traditional (on-ground) students wishing to enroll in an Averett Online course will need to complete this form with their academic advisor & submit to the R.O. for processing. This allows students the flexibility to enroll in a 8-week accelerated course during the traditional 16-week semester.
  • Transcript Request Form
    • Only to be completed if requester is paying via check, money order, or cash. All other transcript requests can be completed by going to www.getmytranscript.com. If you are planning to pay via check, money order, or cash, please stop by Averett Central to fill out the paper transcript request form. One can be emailed to you as well, upon request.   • Withdrawal (W Grade)
    • Will require confirmation from instructor and advisor before processing.
